Georgia Medicaid waivers for home modifications: CCSP, SOURCE & ICWP

Georgia runs three Medicaid home-and-community-based-services waivers that can include environmental modifications — CCSP and SOURCE for older adults and adults with disabilities who qualify for nursing-facility-level care, and ICWP for adults 21–64 with significant physical disabilities. All three require Medicaid financial eligibility plus a functional needs assessment, and modifications are approved case by case as part of a written plan of care, not automatically included.

CCSP — Community Care Services Program

CCSP is Georgia's longest-running Medicaid waiver for people who meet the state's nursing-facility level-of-care criteria but prefer to remain at home. A care coordinator builds a plan of care from a menu of services, which can include environmental modification services — ramps, grab bars, widened doorways — when the plan identifies a specific safety or access need. It's administered through the Georgia Department of Human Services Division of Aging Services and the state's Area Agencies on Aging.

SOURCE — Service Options Using Resources in a Community Environment

SOURCE serves a similar population to CCSP but adds enhanced primary care case management, linking a participant's medical care with their home-and-community-based services. Like CCSP, its service package can include environmental modifications where the care plan identifies the need, alongside services like personal care and home-delivered meals.

ICWP — Independent Care Waiver Program

ICWP is aimed at a younger population: Georgia Medicaid recipients ages 21–64 with significant physical disabilities who would otherwise require hospital or nursing-facility-level care. Its service package is built around supporting independent living in the community and can include environmental modification services as part of an individualized plan of care.

How to apply

For CCSP or SOURCE, start with the Georgia Department of Human Services Division of Aging Services or your local Area Agency on Aging, which coordinates intake and functional assessments. For ICWP, start with the Georgia Department of Community Health. In every case, expect a Medicaid financial-eligibility determination and a face-to-face or telehealth functional assessment before a plan of care — and any modification benefit — is approved.
